Tadalafil is prescribed for:
- Erectile dysfunction (ED) in adult men
- Symptoms of benign prostatic hyperplasia (BPH):
- Difficulty urinating
- Weak urine stream
- Frequent/painful urination
- Pulmonary arterial hypertension (PAH) - improves exercise capacity
Mechanism of Action
- PDE5 inhibitor that enhances blood flow
- For ED: Increases penile blood flow during sexual stimulation
- For PAH: Relaxes lung blood vessels to improve circulation
Dosage & Administration
For ED:
- Two dosing options:
- As needed: Take 30+ minutes before sexual activity (max once daily)
- Daily: Take same time each day (allows spontaneous activity)
- Typical starting dose: 10mg (adjusted based on response)
For BPH/PAH:
- Take once daily at same time
- Do not split tablets
- May require discontinuation of other BPH medications
Important Usage Notes
- Can be taken with or without food
- Does not cure ED or increase sexual desire
- Requires sexual stimulation for ED effect
- For PAH: Controls symptoms but isn't curative
Safety Information
- Not for use with nitrates (risk of severe hypotension)
- May cause headache, flushing, indigestion
- Rare but serious side effects include:
- Sudden vision/hearing loss
- Priapism (prolonged erection)
- Cardiovascular events
Special Considerations
- Alcohol may increase side effects
- Grapefruit products may increase drug levels
- Not protective against STDs/HIV
